7th Grade Vaccine Requirements

Dear Parents of Incoming 7th Graders: NYS Law 2164 requires certain immunizations to enter and attend school. Please check with your health care provider for the Meningococcal Vaccine. NYS Department of Health requires all students in grade 7 to have this. PLEASE NOTE: Students who do not meet this requirement, MAY BE EXCLUDED from school after 14 days of non-compliance, which is on September 27, 2021. Please email a copy of this immunization to [email protected] or fax to 631-673-4609. If our child has already received the vaccine and you have provided me a current immunization form, please disregard. Thank you for your attention to this new immunization requirement.
